Instructions - As an ordained or licensed minister of UCCMA you are required to renew your license annually. To do so you have several options: Option 1 - Renew online - Simply follow the link below, pay your annual renewal offering of $30 and then follow the instructions which will lead you to the online ministerial report form. To renew online, click Here Option 2 - Renew through the mail - Print out the below form and send it with a $30 offering to the address below.
